I, the parent or guardian (please fill in your name below) of the above minor as stated above, give permission to participate in the martial arts session / Birthday party at Wing Chun Bing Fa Kung Fu Academy. If my application is accepted, then in consideration of the Academy I agree to the instructors giving instruction. i) I covenant personally and for my heirs, administrators and executors not to hold the Academy responsible for nor take or make any action, suit, claim demand, cost or expense of any description whatsoever including instructors, students, shareholders or directors and hereby release and forever discharge and agree to indemnify and keep indemnified the Academy. ii) During the lesson, I will obey and abide by all rules, regulations, orders and/or decisions of the Academy its servants, agents, executives and students/ instructors. iii) The above information, which I understand is fundamental to this application, is true and correct in every particular to the best of my knowledge.
